Attached is our first effort at filling out the Event Request Form. Please let me know if additional or different information is needed. The only item we did not fill in was the deadline for acceptance item. We do not have a specific deadline at this point but, clearly, the earlier that we have an answer, the better. However, if you need a date there to force the process to come to closure, we will give you one.
