Action Items:
1. Verify that the electrical heat tracing on the chlorine line from the header trips at 125 degrees C and that the heat tracing is self-limiting.
2. Since a deflagration or detonation in R-1300 is more probable during a start up, evacuation of personnel from the reactor area during start-up should be included in the start-up procedures.
3. Make sure that a leak repair sealant is on the approved sealants list before using it. If it is not on the list, have it tested in the Reactive Chemicals lab. It has been reported that compositional changes have been made in some sealants with just a minor change in the sealant number, which could be overlooked.
4. Replacement of equipment in the chlorine system seldom occurs and Reactive Chemicals training on compatibility of different materials of construction with chlorine is conducted at the Methanes plant annually; however, it was suggested that any new equipment for the chlorine system should be tested for identification of material of construction before it is used.
'5. Consider installing a pressure frequency analyzer in the reactor inlet line to warn of swings in the feed pressure which would indicate an unstable reactor.
