Comments on Statistics--R.T. Hammann There were no Recordable Injuries in September. There has been one recordable injury so far in October where a laboratory analyst cut his left thumb on a broken flask. This brings the number of record able injuries YTD to nine with a total recordable rate of 0.77. Contractor Safety--B.C. Iancaster There was one Loss Work Day Injury in September. A Crescent electrician slipped and fell bruising himself. He was away from work for two days. An accident occurred on October 18 to an Opel laborer while loading trash onto a truck. As he was throwing a 2x4 and some plywood into the truck, a nail cut him in the face requiring 11 stitches. Statistics are attached. D. CEP Safety--B.C. Lancaster There was one Recordable Injury in September. An oiler fell on a man's finger and broke it. This brings the total recordable injuries for the year to 11. Statistics are attached. II. REPORT OF THE SAFETY ADVISORY COMMITTEE (SAC) - G.A. Danner A. Locks-Security--Units/Crafts have not yet depleted their inventory of locks. Therefore, there is no problem at the present time. SHAC needs to establish as a high priority project the development of procedures and policies for handling the lock system within the plant. Hourly Safety Committee 1. Status Report on Previous Concerns a. Dennis Kos spoke to the Hourly Safety Committee concerning NSI handling. 2. New concerns: a. Asbestos in Bags - Asbestos is being collected, put in the paper bags, and placed by the street for pickup. However, these bags are not being picked up and are deteriorating. b. Housekeeping - The feeling was that housekeeping is very poor throughout the plant. B. Plans for Annual Safety & Housekeeping Inspection 1. Theme - SAFETY AWARENESS PREVENTS ACCIDENTS This theme was selected as the majority of our accidents this year have occurred due to inattention and poor awareness on the individual's part. Inspection will be competitive with awards given to the best Unit, Craft, Laboratory and Service Area. 2. Schedule a. Expanded Staff Meeting - 2:00 in the Research Conference Room on October 28. b. Inspection Team is ready and SHAC is ready. The question is -- Is the plant ready? C. Contractor Safety Information developed by Rufus Baca has been reviewed by SHAC. There is a need to form a subcommittee to develop a proposed program. This committee will be: Rufus Baca - Chairman B.C. Lancaster G.W. Bostick R.V. Sharp H.M. This occurred to a Laboratory Analyst who cut his thumb on a broken flask. IV. REVIEW OF INCIDENTS Monsanto Incidents Date Incident No. Description 07/22/83 83-11 (Issued) Recordable Injury--Dept. 14; PT Technician was sprayed with hot PEB while sampling 14D4 feed. Sustained 1st and 2nd degreee burns to left foot. (Wise) 09/09/83 83-15 (Issued) Unit Shutdown--Water in the ben zene feed to alkylator in Dept. 13 resulting in polymer formation. Unit was shut down to clean alkylator. (Moncla) 10/07/83 83-16 (Not Issued) Recordable Injury--Quality Assurance Lab; Analyst cut his left thumb on a broken flask while attempting to seat the flask on a vacuum manifold. (Rolingson) Contractor Incidents Date Incident No. 06/21/83 C 83-03 (Not Issued) Recordable Injury/Days-Away-FromWork--Brooks Construction; Old Dept. 10 area; man was struck by pallet when unloading packing for styrene shutdown. Suffered contu sions and sprain to left ankle. (Wright) V. STATUS OF S-T-O-P PROGRAM--G.W. Daues A. Hourly Program SC 13872 This program will start the week of 11/7 with Mark Riddle as the principal instructor. In addition, there will be hourly participation in the presentation of the material. This will be a 3-unit course with each unit lasting approximately 1-2 hours. Plans are to hold ten sessions per unit to train 700 people. STATUS OF S-T-O-P PROGRAM--G.W. Daues (Contd.) B. Administrative Support The course for non-exempt personnel will also consist of three units. Training for approximately 150 people will start in November with instructors from the Tech Center Safety Committee and Office Safety Committee. Training for this group will be conducted in the ARMS Conference Room. C. Additional Training of Exempt Personnel There are some exempt personnel who have not received S-T-O-P training. Each superintendent will be responsible for training of personnel in their area. VI. MISCELLANEOUS--R.T. Hammann A. Safety and Housekeeping Slides Slides were made available to the superintendents for review with their people concerning the present state of the plant for the Annual Safety & Housekeeping inspection. An additional set of slides are available from Bob Hammann. B. &nergency Procedure Plan A recent S&PP survey emphasized that we need to proceed with our plans to update this manual. Assignments for reviewing and rewriting the manual were made in the May PMSB meeting. Target date for completion of this work is December 31. C. By-Pass of Critical Alarms During the recent S&PP survey it was pointed out that many alarms had been by-passed (i.e., cards pulled, alarms deactivated, etc.). This was addressed during the rewriting of Procedure Bulletin P-0123, but apparently has not received enough emphasis in the plant. A task force will be set up by Tom Ryan, Jack Trafton, Greg Daues and Bob Hammann to review and make recommendations concerning this problem. In addition, a SHAC Alert will be issued using the recent incident of alkylator pluggage as an example. D. Emergency Call-Out List - Procedure Bulletin P-0105 A reminder was given to all superintendents to update this call-out list as needed. sc 13873 STG 2430679 LAM015662 PMSB Meeting Minutes--October 19, 1983 Page 6 E. Review of Recent Incidents 1. A truck carrying a tank from the PE area traveled to the Scale House for weighing. On the return trip prior to leaving the plant, this oversized load bumped a Pumper Gauger pushing him into the tank car loading area. As a result of this incident, all oversized loads from the PE area will be accompanied by an escort vehicle. 2. Steam Incident in Power 2 Problems were encountered with a retractable soot blower located on the #9 boiler in Power 2. In troubleshooting the problem, the lance was activated by electricians. As the lance was entering the boiler, steam exhausted startling the electricians who were who were standing 6-8 feet away. Examination of the assembly after shutdown revealed a crack in the lance. A PM had been performed recently on this soot blower and the crack had not been discovered. MMTS is looking at the metallurgy of the lance. A written report will be issued. VII. PLANT MANAGER'S COMMENTS The Japanese will arrive today with E.N. Brasfield for a visit to AN-5. Gene is anxious to see how the plant looks during the Annual Safety & Housekeeping Inspection next week and will be talking to as many people as possible. Guides should be aggressive and make sure that the inspec tion is snappy and that we keep on schedule.
